Waiver of Liability and Indemnification Agreement for Participation in the Leeds, Utah 5K and 1 Mile Walk/Run Event

Please read this document carefully before signing. By signing this waiver, you agree to the following terms and conditions.
I, the undersigned participant, acknowledge that participation in the 5K and 1 Mile Walk/Run Event ("Event") organized by the Town of Leeds, Utah, is voluntary and involves certain risks. In consideration of being allowed to participate in the Event, I hereby agree to the following:
1. Waiver of Liability: I hereby waive, release, and discharge the Town of Leeds, its officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action, including but not limited to those arising from negligence, that may arise out of or in connection with my participation in the Event.
2. Assumption of Risk: I understand that participation in the Event can be physically strenuous and involves inherent risks, including but not limited to the risk of personal injury, illness, or death. I voluntarily assume all risks associated with my participation in the Event.
3. Medical Advice: I acknowledge that it is my responsibility to consult with a physician prior to participating in the Event to ensure that I am physically fit and capable of participating in such an event. I certify that I am in good health and have no medical conditions that would prevent my participation.
4. Traffic and Course Compliance: I agree to observe all traffic directions and remain on the designated course throughout the Event. I understand that failure to comply with these instructions may result in my disqualification from the Event and potential legal consequences.
5. Indemnification: I agree to indemnify and hold harmless the Town of Leeds, its officers, employees, volunteers, and agents from any and all claims, demands, actions, or causes of action, including but not limited to those arising from negligence, that may be brought by me or any third party in connection with my participation in the Event.
6. Age Verification: I certify that I am at least 18 years of age. If I am under the age of 18, I certify that I have the consent and authorization of my parent or legal guardian to participate in the Event.
By checking the acceptance box, I acknowledge that I have read and understood this waiver and indemnification agreement, and I voluntarily agree to its terms.
